The differences are minor. So long as you mention it in negotiation I can freshen up beforehand. If you want to be on the receiving end, clean yourself out like you're getting ready for a prostate massage, mention it in the negotiation, and clean the area well in the shower and it's no big deal! Oral sex is oral sex in my book.

Isn't it DATO? Anyway, I haven't paid any extra for it. However, many ladies will not partake (even just receiving). Any a large number will insist on a dental dam or similar. Your bigger problem will be finding a lady that allows it. I've asked similar questions in the past if you want to check responses there. Price wise though, shouldn't be a difference.
